Home Care is a successful means of providing high quality health care services to the patient and their family in the home environment. The need for services will be determined by the patient's physician, in conjunction with the professional nurse coordinator, the patient and the family. The services provided by our Home Health Agency bring to your home the same professional care you receive at Brookhaven Memorial Hospital Medical Center. The programs offered are available to any patient residing in Suffolk County and patients may range from infants to the elderly.

These services may help to shorten or prevent an inpatient stay in the hospital, provide a smooth transition from the hospital to the home, prevent or delay placement in a nursing facility, and reduce the stress on the patient and family.

There are two categories of home care associated with the Home Health Agency:
Certified Home Health Care
Long Term Home Health Care


Community Health Nurses are available 7 days a week including holidays, 24 hours a day. Social Workers, Rehabilitation Therapists, and Home Health Aides are also available seven days a week.

Referrals Referrals and requests for Home Health Agency services may come from physicians, nurses, social workers, or discharge planners, community agencies, family members, as well as the patients themselves. Once the referral is received, a member of the Home Health Agency staff will obtain the necessary information and speak with the physician, patient and family to determine what services are appropriate and necessary.

Technology Offers Prolonged Independence
Brookhaven Memorial Hospital's Home Health Agency now offers the Telehealth program which allows Home Care nurses to monitor vital signs as well as see and speak with patients from a distance, greatly improving efficiency and continuity of care. Learn More >

Financing for Home Care
Brookhaven Memorial Hospital Home Health Care Agency's services may be paid for by Medicare, Medicaid, Blue Cross, Worker's Compensation, No Fault Insurance, as well as many other health insurance plans. In most cases, services are billed directly to the insurance company, with no expenditure required on the part of the patients' family. A home care staff member will discuss your insurance benefits prior to initiating services. In some instances, patients may pay for services privately, on a sliding fee scale, or receive them without charge, when no other resources are available.
